Paul Traub Associates has practical experience of applying Human Factors to Nuclear Projects in a thorough (yet pragmatic) manner. Examples of our project experience is summarised below.

Dreadnought Site Safety Case

Human Factors Lead for all aspects of the Successor Site Safety Case. This includes provision of Human Factors Integration Strategy for a number of activities on the Barrow Nuclear Licenced Site. Reviews of claims on operator safeguards and detailed Human Reliability Assessments including claims on Alarm responses were part of this work. Work included probabilistic assessment of crane collapse.

Defence Nuclear Safety Committee - Human Factors Specialist

Human Factors Specialist Member of the Defence Nuclear Safety Committee (DNSC). The DNSC advises the Secretary of State for Defence (and other ministers) on all public safety matters relating to the construction, operation and maintenance of nuclear powered warships and the design, manufacture, transport, storage, handling and operational training related to nuclear weapon systems. DNSC is an advisory non-departmental public body of the MoD.

Onet - Design Support for a Jetvac System for Removing Low Level Waste at Devonport

This work involves providing guidance on the design of the control panel, access, reach and alarm management for this system. The scope of work also includes manual handling, hose carriage assessments and guidance on valve design.

EDF- Independent Review of Human Error Probabilities Derived for Torness PSR Safety Case

Paul reviewed a number of HEPs that were derived for the Torness Safety Case using HEART and NARA. The review found that dependency and that the interaction with THEP had not been fully addressed. As a result of the review a number of the HEPs were modified resulting in subtle changes to the PSA model.

Rolls Royce Marine Power - Derivation of Human Error Probabilities for Successor Programme

Derivation of a large number of HEPs in support of the Submarine Successor Programme PSA. This includes HEPs for LOCAs and Intact Circuit Faults and detailed Human Error Identification. An Associated HFIP to support the safety case for PWR3 was also produced for this work.

Babcock Marine
Magnox - Derivation of Human Error Probabilities/Support to HAZID

This work was undertaken in support of a PSR. A number of HEPs were derived using HEART and THERP. An additional activity was to be the Human Factors expert for a sludge facility HAZID.

Atomic Weapons Establishment
Dounreay

A detailed Human Factors peer review of a client's report supporting a waste processing safety case was undertaken. The review assessed the findings of the Human Factors Walk-down and also reviewed and updated the Human Error Probabilities that had been produced to support the safety assessment. As a result of the review a number of claims on operators and proposed safeguards were modified so that site safety could be improved.
